5. Add State and Zip*

6. Add Choose Insurance*

3. Tap “Add Patient”

4. Tap “+” to add the supply given to the patient.

5. Tap “Add to Order”

6. Tap the “I hereby acknowledge” and “Proof of Delivery: I have…” checkboxes; both are required. Tap on the dropdowns (where the yellow circle is) so the patient can read the statements

7. Tap here to print the delivery ticket for the patient to review.

8. Have the patient sign here.

9. Click “Finish”
